What is the NDIS?
The National Disability Insurance Scheme (NDIS) is an Australian Government initiative designed to provide support and funding for people living with a permanent and significant disability. The scheme helps participants access the services and supports they need to live more independently, achieve their goals, and actively participate in their communities.
The NDIS is built around the principle of providing individuals with greater choice and control over the supports they receive. Rather than relying on a one-size-fits-all approach, eligible participants receive funding tailored to their unique needs, enabling them to access appropriate services and supports that enhance their quality of life.
The NDIS is administered by the National Disability Insurance Agency (NDIA), which is responsible for assessing eligibility, approving access requests, and determining the funding participants receive. The NDIA operates on behalf of the Australian Government and is funded through the Department of Social Services (DSS).
NDIS Eligibility Requirements
To be eligible for the NDIS, you must meet the following criteria:
Be under 65 years of age when applying.
Be an Australian citizen, permanent resident, or hold an eligible visa.
Have a permanent and significant disability.
Require support or assistance due to the impact of your disability on everyday activities.
Live in Australia in an area where the NDIS is available.
If the NDIS is not yet available in your location, you may need to explore alternative options until access becomes available.
Individuals aged 65 years and over are generally supported through Australia's aged care system rather than the NDIS.

Who Decides if I Can Access the NDIS?
The National Disability Insurance Agency (NDIA) is responsible for determining whether you are eligible to access the NDIS and what supports are considered reasonable and necessary based on your individual circumstances.
If you believe you may be eligible, you can submit an Access Request through the NDIA.
Steps to Access the NDIS
Step 1
Complete the NDIS Access Checklist to determine whether you meet the basic eligibility requirements.
Step 2
Contact the NDIA on 1800 800 110 to request an Access Request Form.
Step 3
Review the NDIS Access Process and gather any supporting evidence required to support your application.

Providing advance notice helps us make appropriate arrangements and continue delivering quality support services.
Service Changes Initiated by New Leaf Support Care
Should there be any changes to your scheduled support service, such as your regular support worker being unavailable due to illness, leave, or unforeseen circumstances, we will notify you as soon as possible.
Notifications may be provided via:
Phone call
SMS/Text message
Email
If you prefer an alternative method of communication or would like another person notified on your behalf, please let us know. Wherever possible, we will arrange a replacement support worker or alternative service to minimise disruption to your supports.
Cancellation Policy
To ensure the sustainability and availability of our services, New Leaf Support Care has a cancellation policy in place.
A cancellation fee may apply when:
A scheduled service is cancelled with less than 24 hours' notice; or
A participant does not attend a scheduled service without prior notification.
In accordance with current NDIS Pricing Arrangements and Price Limits, a late cancellation fee of up to 90% of the agreed service cost, including associated travel charges, may be charged.
Appealing a Cancellation Fee
We understand that exceptional circumstances can arise. If you believe your situation warrants special consideration, you may request a review of your cancellation fee.
To submit an appeal, please complete our Cancellation Fee Review Form. Each request will be assessed on a case-by-case basis.
Changes to Your Supports
We recognise that your goals, needs, and preferences may change over time.
If you would like adjustments made to your Support Plan or the services you receive, please speak with our team. We will work closely with you to understand your requirements, update your support arrangements, and ensure all changes are documented appropriately within your plan.
Termination of Services
New Leaf Support Care is committed to providing reliable and consistent support services to all participants.
However, repeated cancellations, ongoing non-attendance, or failure to comply with agreed service arrangements may result in:
Loss of preferred service times;
Changes to service availability; or
Termination of services following appropriate communication and review.
Where possible, we will work with participants to resolve issues before considering service termination.
